Box to enter credit card information is truly missing

I've already seeen the post where the credit card information box is grayed out, but does work.  However, in our case,  it's not grayed out - it's non-existent.  There is no space provided where the user can enter the credit card number.  I could change the country or enter a gift card, but not regular payment information.  Here is the payment page:

Can you provide a link to a product on your website? I couldn't find one to test your checkout page. It might be an issue with the older theme you're using, if I had to guess.

Thank you, Adam, for your input. As you suggested, I changed the theme.  However, when the Secure Checkout page is displayed, there is still no opportunity to enter credit card information.  Plus, the theme change has given me lots of formatting issues to clean up.  Sheesh...  But thank you for trying.

Hi @SueP I'm also not able to find a product on your site to view the checkout page. Can you post a link to one of the items you are testing?
