Is there any way to add a form to collect basic demographic information when clients book a service by clicking on the button embedded in my separate non-Square website?
Being able to add a form or demographic information to the booking process would still be considered a Feature Request for Square Appointments.
This is not currently planned by our Product Team for implementation, but it doesn't mean it's could not be added in the future. I have moved your post to our Ideate Board for Appointments. Here other sellers can add their voice to the request to show the Product Team how popular it is among our sellers. Once more information becomes available, a Community Moderator will circle back to update the status to keep you in the loop.
